Application Process
The application package consists of three documents:
1. MCCCD application to be submitted online only
2. AZPOST Medical Waiver form (POPAT form)
The AZPOST Medical Waiver form must be signed by a physician before submitting. It is required by any applicant who does not have an active AZPOST certificate or who has an active AZPOST certificate but has not been actively employed with an Arizona law enforcement agency for three or more years.
3. AZPOST Statement of Personal History form
The AZ Statement of Personal History form must be notarized on page 2 before submitting. If a candidate is recommended for hire, a background investigation will be initiated, based on this form and other documentation as requested by the program director.
The AZPOST forms must be delivered separately by mail or in person. They cannot be faxed or emailed and are unable to be submitted online.
